New Shipping Policies for Alaska, Hawaii, and Canada!
Alaska and Hawaii: Effective September 1, 2006 FedEx return shipping materials will be included in all kit orders. No additional shipping charges will apply. Canada: Effective October 1, 2006 FedEx return shipping materials will be included in all kit orders. Canadian customers will be receiving a new fee schedule to reflect new rates inclusive of shipping. To request a new fee schedule or FedEx shipping materials for kits you may currently have in stock, please contact us at 1-800-221-4640 or click here to place a request online.

Forms and Instructions
Shipping Information
Metametrix provides its clients with specimen collection kits for tests. Each kit comes with detailed specimen collection instructions.
Please note the following important points:

Send the specimens in shipping containers that come with the test kit. This helps expedite the handling and processing of the specimens.

All samples received by Metametrix must be properly identified. Clearly label each specimen with a permanent marker: patient’s full name, date of birth, collection date, and specimen type.

Specimen integrity is essential. Properly position tubes so they are secure in the mailing containers before inserting them in the mailer.

The tops on all tubes should be securely sealed prior to shipping to help assure the safe arrival of specimens and prevent delays of the test results.

Send specimens via FedEx®, DHL or Purolator, Monday through Thursday only. To schedule a pickup with FedEx call 1-800-GO-FEDEX (1-800-463-3339) or visit their website at www.fedex.com.
Testing may also be done without the use of a Metametrix supplied kit. A completed Test Requisition Form must accompany the specimen or the test cannot be processed. Please call client services to have a Test Requisition faxed to you. You may use a carrier such as FedEx® or DHL® to ship specimens back to our laboratory. Be sure to follow the specific test collection and shipping preparation instructions for appropriate handling of specimens (refer to test specific instructions in this guide).
Additional Charges
Metametrix incurs additional expenses under the following circumstances and will request additional payment for:
- Resubmittal (submittal of a new specimen to replace an unacceptable one): $15.00
- STAT requests: $25.00 per profile. $50.00 for ION or Cardio/ION.
- Modification of billable stamp: you will be charged if you make a change on the billable stamp that increases FedEx® charges to the laboratory.
- ION, Pediatric ION, and Cardio/ION Profiles: $50 charge when urine and blood specimens are not received in a single shipment.
International Shipping of Specimens
Instructions
- Please review the specimen collection instructions in this guide for the specific test before collecting the specimen.
- Follow the instructions for preparing and packaging the specimens.
- Include the following in the shipment to Metametrix:
- Specimen(s)
- Completed test requisition
- Payment in full
- The party ordering the test is responsible for the cost of shipping specimens to Metametrix.
- Specimens should arrive at Metametrix in no more than 3 days; overnight is preferred.
Legal and Customs Requirements

Select a reputable carrier who observes the required legal and customs issues for shipping biological specimens. Use FedEx®, Purolator®, DHL®, or a carrier of your choice.

Check with your carrier to identify the specific documentation required to ship your biological specimens to the U. S. Typically, carriers will require completion of a Commercial Invoice Form.

Follow the carrier’s instructions for completion of the airway bill to transport clinical specimens.

Package the specimens according to the carrier’s instructions.

Affix customs documentation and the airway bill to the outside of the package so the carrier can read it.
