Who Can Order Testing?

Only healthcare providers licensed in their state may order laboratory testing. MDs and DOs are certified in all 50 states. If you are unsure of regulations in your state, please contact your governing or licensing board for assistance. If you are still unsure or not able to order testing, providers have the option of ordering through an authorized physician. Please contact Client Services at 800-221-4640 for details on how to set up an account through an authorized physician.

New Healthcare Providers

• Click "Register" to submit your contact information and request testing information.
• Download, complete, and return the “New Physician Account Agreement”. Please note, New York and International healthcare providers have separate account agreements.

• Call Client Services at 800-221-4640 for your personal healthcare provider login to order kits and literature online or place your order directly over the phone. Please note, international healthcare providers (excluding Canada) must keep a valid credit card on file to cover kit shipping charges to international destinations.
• Collect specimen and ship with payment via FedEx using the shipping materials provided (US and Canada). Shipping from US territories or other international entities is not provided by Metametrix.

How to Complete the Test Requisition Form

A Test Requisition Form is included with each specimen collection kit. The form indicates which test is to be performed and includes pertinent information about the patient and ordering physician. Print the “Testing Request for Your Clinician" form below to provide ordering information for your healthcare provider. Your healthcare provider must have an account with Metametrix or be willing to open one to order testing. If you prefer to find a healthcare provider already offering Metametrix testing services in your area, please call Client Services at 800-221-4640 or email findadoc@metametrix.com.

PLEASE NOTE: This form is NOT for ordering testing. Please use this form to request information about Metametrix testing. Any specimens submitted with this form without a valid account will not be processed.
